An exciting opportunity exists for an energetic and dedicated Social Worker or Occupational Therapist to join the Transcend Outreach team
providing individualised goal oriented support to people who have a mental illness.
Transcend’s Outreach Program is a state funded, recovery focused initiative providing guidance and support to people whose lives are severely affected by mental illness.
The Outreach Support Coordinator will be responsible for:
· Intake and assessment of case load;
· Development and implementation of recovery plans in a person centred planning framework;
· Network with local service providers.
In addition to an appropriate qualification (or working towards qualification), the successful applicant will have experience in the provision of support services to people with mental illness, knowledge of the local community and support mechanisms and linkages for people with a mental illness. This position has the potential to increase to 0.6 EFT.
